Job Description:

The primary responsibility of the Dietary Staff is to prepare and provide delicious, nutritional meals for our patients and to keep the dining and kitchen area in clean and pristine condition

Job Duties and Responsibilities:

• Prepare meals for patients according to their dietary specifications
• Must be knowledgeable of food procedures
• Must be able to cook a variety of foods in large quantities
• Must maintain the care and use of supplies, equipment, the appearance of work areas, and perform regular inspections of food service areas for sanitation, order, safety and proper performance of assigned duties.

Education:

• High School diploma or GED

Experience and Specific Requirements:

• Would prefer that applicant have at least 3 years food preparations experience in a health care facility, should be 18+ years old
• Ability to work in a fast paced and sometimes stressful environment while maintaining a calm demeanor
• Must be able to read and take instructions and follow directions
• Must be able to work both as a team and independently
• Must have good communication skills and be able to read, write and speak the English language
• Must be able to bend, reach, stoop, lift minimum of 25 lbs

• Oversee all areas of the patients medical needs on a daily basis
• Work in collaboration with patient, physician, family members, care team and other healthcare professionals to ensure the best possible care and treatment for the patient
• RN/LPNs-pass meds, provide treatment, assess for changes in conditions
• Answer call lights, assist patient with ADLs
• Responsible for timely and accurate charting
• Do whatever is takes to make our patients happy with their stay

• 1 year of experience in a Skilled Nursing Facility is preferred
• Possess the ability to make independent decisions, follow instructions and accept constructive criticism
• Possesses leadership ability and the willingness to work harmoniously with other personnel-fosters a true team environment
• Demonstrates judgement and has the ability to maintain highly confidential information

Job Description: The primary purpose of this position is to assist in planning, organizing, developing and directing Physical Therapy Services in the facility in accordance with current applicable federal, state and local standards, guidelines and regulations, and as may be directed by the Administrator, to assure that the highest degree of quality resident care can be maintained at all times.

Job Duties and Responsibilities:

• Assist in standardizing the methods in which therapy will be accomplished.
• Participate in discharge planning, development and implementation of resident care plans, resident assessments, etc., as necessary.
• Maintain treatment records, resident files, and progress notes as required.
• Ensure business office receives charges for therapy services rendered to residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
• Assist in interviewing residents to evaluate daily living skills and capacities and determine extent of abilities and limitations.

Experience and Specific Requirements:

• Must have a minimum of two years experience in a therapy assistant capacity in a hospital, nursing care facility, or other related medical facility
• Must have training in rehabilitative and restorative therapy practices.
• Must have a current, unencumbered, active license to practice as a Physical Therapist Assistant in the state of Texas.
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
• Must be knowledgeable of physical therapy procedures.

Job Duties and Responsibilities:

• Must be able to perform Speech Therapy Evaluations, complete care plans, establish goals, and communicate frequently with interdisciplinary team
• Must utilize good body mechanics at all times
• Must be able to make safe, independent decisions when circumstances arise
• Must be knowledgeable of speech therapy practices and procedures as well as the laws regulations
• Must be able to interpret and implement the programs, goals, policies and procedures of the speech therapy dept.
• Maintains professional working relationship with all families, peers, associates, vendors.
• Understands and complies with company policies, maintains current status with Relias training.
• Documentation is completed daily including therapy notes, treatment times and labor hours
• Must be able to assist in the evacuation of patients
• Must demonstrate dependability and regular attendance
• Must have excellent customer service, clinical skill, and communication and positive attitude
• Be able to lift a minimum of 35 pounds, safely transfer patients, participate in staff training related to safe patient handling.
